The passive uses worden + participle (present / simple past) and zijn + participle (perfect):

The agent is introduced with door: Het boek is door Mulisch geschreven.
Watch out: Het huis is gebouwd means "has been built" (perfect passive), not "is built" as a state.
Er-passive without a subject: Er wordt hier niet gerookt. — No smoking here.
